Love handles are the areas of soft tissue that sit over the flanks and upper hips, forming a visible bulge when clothing is tight. For many patients they are the last remaining area of resistance after considerable weight loss, extensive exercise, or years of stable body composition. The tissue involved is usually a genetically determined fat deposit that behaves differently from fat elsewhere on the body, responding poorly to calorie restriction and remaining visible even in patients who are otherwise lean. Understanding that distinction is what determines whether body contouring is the right answer, and which combination of techniques will actually work.

What Causes Love Handles

Three factors typically contribute. The first is genetic fat distribution, where the body preferentially stores fat in the flanks and lower back regardless of lifestyle. The second is age-related change, as metabolism slows and hormonal shifts through the thirties, forties and beyond cause fat to redistribute towards the midsection. The third is skin quality, which can leave the flanks looking soft and undefined even after fat has reduced, particularly in patients who have lost a large amount of weight. Most love handles involve two or more of these factors, which is why the treatment plan is built around the individual rather than a fixed protocol.

Why Training and Dieting Rarely Shift Them

It is common to meet patients at consultation who have already put years into training and careful eating without seeing their flanks change. That is not a failure of effort. Genetically determined fat deposits are metabolically resistant, and general fat loss cannot be directed towards a particular area of the body. The same limitation applies to targeted exercise. Working the oblique or lateral abdominal muscles does not reduce the fat sitting over them, because fat loss happens across the body rather than in the area being trained. Knowing this before choosing a treatment matters, because it explains why so many people plateau despite doing everything right.

The McKeown Medical Body Contouring Combination

Historically, treatment for stubborn love handles meant traditional liposuction, which addressed the fat but did nothing for skin quality or contour refinement. Body contouring technology has moved on considerably since then. At McKeown Medical Glasgow, treatment for the dad bod and wider waistline concerns combines three complementary technologies into a single comprehensive approach. McKeown Medical is currently the only clinic in Scotland offering this combination of technologies.

Treatment is performed by Dr Kavita Sharma (GMC 7039911), Consultant Plastic Surgeon, who is on the GMC Specialist Register. Each technology targets a different part of the problem, which is why the combination is more effective than any single treatment used in isolation.

Ultrasound Assisted Liposuction (UAL)

Ultrasound Assisted Liposuction is a technique that uses controlled ultrasonic energy to emulsify fatty tissue before it is removed. For love handle liposuction this carries a particular advantage over traditional approaches. Ultrasonic energy breaks down fat cells at a cellular level, which allows it to address the dense, fibrous fat deposits typical of the flanks without the mechanical force older techniques required.

The flank area is fibrous by nature, particularly in male patients, and traditional liposuction can struggle to release this tissue cleanly. Ultrasonic energy softens the fat before extraction, allowing more precise removal and less trauma to the surrounding tissue.

Power Assisted Liposuction (PAL)

Power Assisted Liposuction (PAL) uses a specialised vibrating micro-cannula to precisely contour fatty tissue. Where Ultrasound Assisted Liposuction releases and emulsifies the fat, PAL refines the final contour with a level of precision that hand-driven techniques cannot achieve.

On the flanks specifically, PAL follows ultrasonic emulsification and is used to shape the waist so that it flows naturally into the torso above and the hip below. Getting that transition right is what separates a contoured waistline from an obviously treated one, and PAL is the technology that delivers it.

Quantum RF Skin Tightening

Quantum RF Skin Tightening is the third element and addresses the skin quality component of the contour. This is an advanced radiofrequency technology that uses controlled RF energy to contract collagen fibres, tighten loose connective tissue, and stimulate collagen remodelling over the months following treatment.

Skin quality is the reason fat removal alone often disappoints on the flanks. Take the fat away without treating the skin above it and the result can be a soft, undefined waist where a sharper one was expected. Because Quantum RF works beneath the skin rather than at the surface, it produces structural tightening rather than a surface effect.

Why the Three Are Combined

Each of the three answers a different problem, and any one of them used alone leaves something untreated. Fat removal without skin tightening can leave residual laxity. Skin tightening without fat contouring cannot resolve the underlying deposit. Contour work without ultrasonic release can produce an uneven result in the fibrous tissue typical of the flanks. Planning all three into a single treatment allows the fat to be removed, the waistline refined and skin contraction improved together, producing a smoother and more natural contour than one treatment alone would typically achieve.

Matching the Treatment to the Cause

Because love handles usually involve more than one factor, the balance between the three technologies shifts from patient to patient.

  • Discrete fat deposit with good skin quality: ultrasonic emulsification followed by PAL contouring, with no tightening stage needed
  • Dense, fibrous flank fat, more common in male patients: heavier emphasis on the ultrasonic stage, which releases tissue that resists mechanical extraction
  • Skin that has lost elasticity following weight loss: Quantum RF carries more of the work, sometimes alongside only modest fat reduction
  • Most patients: all three in some proportion, with the balance set at consultation rather than decided in advance
  • Substantial skin excess: surgical body contouring is likely to be the better answer

Who the Treatment Is Suitable For

The McKeown Medical combination suits patients who are near their ideal weight but retain stubborn flank or waist fat, who want improved waist definition, who have mild to moderate loose skin over the flanks, or who want to avoid more invasive surgical options. It is suitable for both male and female patients, with the technique adjusted to the tissue being treated.

What Happens During Your Consultation

Assessment begins with fat distribution across the flanks and lower back, the quality and elasticity of the overlying skin, and how the waist sits in relation to the rest of your torso. Those findings determine which of the three technologies are needed and in what proportion. Realistic expectations are discussed at this stage, including what the treatment can and cannot achieve. Consultation carries no obligation to proceed.

The clinic is regulated by Healthcare Improvement Scotland, and any patient considering a surgical or minimally invasive procedure can check a practitioner’s registration and specialty directly on the GMC register.

Frequently Asked Questions

How to get rid of love handles without surgery?

For patients with mild to moderate love handles, the McKeown Medical combination of Ultrasound Assisted Liposuction, Power Assisted Liposuction, and Quantum RF skin tightening can produce meaningful improvement without formal surgery. Where there is large volume or substantial skin excess, surgical body contouring may deliver a more comprehensive result.

How do men get rid of love handles?

Male love handles tend to involve denser, more fibrous fat than female love handles, which is why Ultrasound Assisted Liposuction is a valuable part of the approach. The same combination applies to both, with the technique adjusted to the tissue being treated.

Can you lose love handles without losing weight everywhere else?

Yes, and this is the central advantage of localised body contouring over general weight loss. Diet and exercise reduce fat across the whole body, which means genetically resistant deposits like the flanks are usually the last to shrink. Targeted body contouring addresses the flanks specifically without changing body weight elsewhere, achieving a change in shape that general weight loss cannot.

How long does recovery take?

Most patients return to normal activities within a week, with some swelling and bruising in the first days that settles steadily. The refined result continues to develop over the following months as collagen remodelling progresses.

Are love handles permanent after treatment?

Fat cells removed during treatment do not return. Results are long-lasting provided weight remains stable, since substantial weight gain can cause remaining fat cells in other areas to enlarge and alter the overall contour.

Does the treatment work for both men and women?

Yes. The combination is effective for both, though the specific approach is tailored to the individual pattern of fat distribution and skin quality.

Who performs the treatment?

Treatment is performed by Dr Kavita Sharma (GMC 7039911), Consultant Plastic Surgeon, a member of the British Association of Aesthetic Plastic Surgeons and on the GMC Specialist Register.

A Note on the Right Approach

No two waistlines are the same, and effective treatment starts with understanding why yours has changed. Fat distribution, skin quality and overall body shape are all assessed before anything is recommended, and the aim is always the least invasive route capable of producing a result worth having.

If stubborn flank fat, a soft waistline or a shape that has not responded to diet and exercise is the concern, a consultation at McKeown Medical Glasgow can establish the cause and whether body contouring is the right answer for you.

Some more Quantum Lift magic!

This patient is just 24 hours after her Quantum lift with fat redistribution. As part of the procedure we sometimes take some of excess fat that’s been removed from the neck and jowls and replace it along the chin and jaw to create more definition.

Although she’s still bruised and swollen, I think we can all already see where this result is heading.

The patient is delighted! What do you think?
Dr Rhona is back!

We’re delighted to share that Dr Rhona Cameron will be returning to clinic at the beginning of September following her maternity leave and her diary is now open for appointments!

We know many of you have been looking forward to seeing Rhona again, and we’re so pleased to welcome her back!

Once again, the combination treatment wins the day!

This patient came to us hating her neck and jowls, but she would not consider anything invasive. Treating the neck is always a challenge especially with non-invasive treatments only.

For this patient we first of all reduced the fat under the chin using CoolSculpting, then we did some hyaluronic acid under the mandible to define the jaw and finally we did some Sofwave to tighten the skin.

The patient is over the moon with her results. What do you think?
I genuinely think the Quantum Lift is changing what is possible for patients who want to improve their jawline and neck, but do not want to undergo facelift surgery.

I have used earlier generations of skin-tightening technology and, honestly, I was often underwhelmed by the results. This is different.

By combining precise power-assisted liposuction, targeted treatment of the neck muscle where needed, and QuantumRF skin tightening, we can address the three things that most commonly age the lower face and neck: muscle, fat and skin.

This patient is shown just 24 hours after treatment. There is still swelling to settle, but the early change in her jawline and neck contour is already clear.

I have also included clips from the procedure for anyone who likes seeing the technical side of what we do.

Would you consider a minimally invasive alternative to facelift surgery for your jawline or neck, or would you rather go straight to surgery?
