Deep plane face & neck lift

A Deep Plane Face and Neck Lift is the most advanced form of facelift surgery available today. Rather than simply tightening loose skin, it restores the deeper facial structures to their original position, redefining the jawline, refining the neck, and rejuvenating the lower face in a way that looks entirely natural. At McKeown Medical in Glasgow, we offer the Deep Plane Face and Neck Lift for patients seeking meaningful, long-lasting improvement without looking operated on. The aim is not to change your face, but to restore it.

This approach focuses on anatomical precision, allowing for smoother contours, improved facial balance, and results that age naturally over time with consistent, refined support.

Deep Plane Face & Neck Lift Overview

A Deep Plane Face and Neck Lift is suitable for men and women experiencing visible lower facial ageing where non-surgical treatments are no longer sufficient.

By repositioning deeper tissues rather than tightening skin, this advanced face cosmetic surgery procedure delivers natural rejuvenation that continues to age gracefully.

Choosing Deep Plane Face & Neck Lift

A Deep Plane Face and Neck Lift is chosen when structural ageing is the primary concern. It is most appropriate for patients with visible jowls, jawline softening, and neck laxity that cannot be improved to the extent you desire with non-surgical treatments.

The decision is not about age. It is about anatomy. When tissue descent is significant, a Deep Plane Face and Neck Lift offers the most reliable and long-lasting correction.

Deep Plane Face & Neck Lift vs. Similar Procedures

Not all facelift techniques address ageing in the same way. Traditional approaches often tighten the skin or adjust the superficial muscle layer. While this can improve laxity, it does not fully correct deeper structural descent.

A Deep Plane Face and Neck Lift works beneath the SMAS layer, releasing retaining ligaments and repositioning the facial tissues properly. This reduces tension, preserves natural movement, and produces results that age more gracefully.

Discover who makes the best candidate for a deep plane facelift and why this advanced surgical technique delivers long-lasting, natural-looking results. Learn about the key factors for success, the importance of good health, and why nicotine consumption is a major contraindication.

Transcript

The best candidates for a deep plane facelift are people who have moderate to severe facial sagging, especially around the jawline, the jowls, and the neck. To be a good candidate, you also need to be in good general health and have a desire for long-lasting natural results, and understand that to achieve that outcome requires an advanced type of procedure. The main contraindication to having a deep plane facelift is nicotine consumption through either smoking or vaping, or any other nicotine products. Nicotine constricts blood vessels, which impairs the healing ability of our tissues, which is crucial to achieve a safe result.

How long does a facelift last?

Facelift results typically last 5–10 years, but deep plane techniques offer longer-lasting rejuvenation. Find out how skincare, injectables, and sun protection help maintain your youthful look over time.

Transcript

The most common answer to how long does a facelift ask is 5-10 years, which is not a bad estimate. However, the concept of results longevity is a little more nuanced than this. After you’ve had a facelift procedure, particularly a deep plane face and neck lift, the excess tissues in the neck are reduced, and the remaining tissues in the face and the neck have been redraped into a more youthful position. Although you will, of course, continue to age, you will always have less excess tissue and a more youthful appearance than if you’ve never had the surgery. In that regard, there should be a more permanent improvement in your appearance. It’s a good idea to maintain your results with good skincare, including sun protection, injectables, and laser treatments, to maintain your skin quality.

Deep plane facelifts offer longer-lasting, more natural results by lifting and repositioning deeper facial structures rather than just tightening the skin. Learn how this advanced technique provides a youthful look that ages gracefully over time.

Transcript

Deep plane facelifts last longer than traditional facelifts because they lift and reposition the deeper facial structures rather than just tightening the skin. By releasing and repositioning the deeper tissues, this technique provides a more stable and longer-lasting result, reducing the risk of skin sagging again too quickly. When you’ve had a deep plane facelift, although you will continue to age, you’ll always look younger than if you’d never had it. Additionally, because the tension is placed on the deeper layers rather than the skin, the results appear more natural and age gracefully over time. Proper skincare and a healthy lifestyle can further extend the longevity of the facelift.

Transcript

Deep plane facelifts are safe when performed by an experienced surgeon. This advanced technique provides long-lasting results by lifting the deeper facial structures rather than just tightening the skin. To do this, we use a painstaking technique to dissect the deep plane with some very delicate instruments to protect all the facial nerves and operate safely within this space. After surgery, you will have very thorough aftercare to make sure that you get through the healing period as smoothly as possible. With expert technique and proper aftercare, deep plane facelifts offer safe, long-lasting rejuvenation with a smooth recovery.

Transcript

A well-performed facelift should look natural, enhancing facial contours without appearing to look tight or pulled. The most highly skilled facelift surgeons achieve this by performing the deep plane facelift technique. With the deep plane technique, we go deeper underneath the soft tissues of the face, releasing the retaining ligaments that hold the face in place and allowing us to reposition the tissues rather than simply placing tension on them. The deep plane technique is a much more complex and sophisticated type of facelift. Although there is now a consensus amongst the world’s best facelift surgeons that this is the gold standard method if you want to achieve the most natural results that last a long time.

What is my recovery like after a deep plane facelift?

Summary

Recovering from a deep plane facelift is a gradual journey. While the initial swelling and bruising peak within the first few days, most patients feel comfortable resuming social activities within 2–3 weeks. Subtle swelling continues to improve over the following months, with full healing and scar maturation taking up to a year. However, many feel “camera-ready” for major events within 4–6 months.

Transcript

The recovery from a deep plane facelift is a bit of a journey, and it’s important to know what to expect from the outset. When you wake up from your procedure, most patients are already impressed by the degree of improvement they can see. The jawline looks very defined, and the neck is considerably improved. However, your face will be bruised and swollen, and this typically gets a bit worse over the first few days before slowly starting to get better again. Usually, by around two or three weeks post-op, the bruising and swelling will have subsided to the point where you feel comfortable getting back to most social situations, like going back to the office. The remaining subtle swelling will improve as the weeks and months pass by. Although it can take up to a year for full healing and scar maturation. Most patients, however, will feel comfortable and camera-ready for big events like weddings in around four to six months.

Liz, 60, opens up about her decision to undergo a facelift, liposuction under her chin, and dermal fillers to restore her youthful appearance. Tired of looking worn out, she wanted a refreshed but natural look. In this video, she describes her consultation process, overcoming fears of surgery, and the compassionate care she received from expert surgeons. Her results were so natural that even close friends couldn’t pinpoint what had changed, just that she looked incredible! With minimal scarring and newfound confidence, Liz says she would do it all over again in a heartbeat.

Transcript

Hi, my name’s Liz. I’m 60 year old and had surgical facelift, lip suction under my chin, and surgical and derma fillers. I was looking tired all the time with bags under my eyes, a double chin, and deep lines around my mouth. I felt self-conscious and unhappy with my appearance. I decided that for the sake of my own self-esteem, I would like to give nature a helping hand. But at the same time, I didn’t want to look unnatural. I was worried about the surgical part and whether I could cope with not being put under anaesthetic. I knew I would be awake, and a local anaesthetic would be used. My initial consultations were fabulous, and I was put at ease by Tom, who was my first contact with the clinic. He listened carefully to what I didn’t like about myself and was very patient with all myself and my partner’s questions and guided us through the options open to me. Once I decided that I wanted to take things further, I had a consultation with Dr Russell, and he went through the procedures with me. Once again, I felt at ease. I didn’t feel at all pressured. My consultation with Darren was also very relaxed, and I felt totally reassured that I was in the hands of a professional and caring company. I felt that I was well informed of the possible risks, but I felt that I was in safe hands anyway. The procedures went well, even though I was quite anxious about the surgical part. I was kept occupied with the lovely staff, and they were always talking and encouraging me throughout my time in surgery. That also applied when I was having the derma fillers after I had healed from the surgery. Darren was so nice. He made me feel so relaxed while chatting to me and injecting the fillers at the same time. I didn’t even have time to feel anxious. I wouldn’t say there was no pain at all. There was a little discomfort, but it was relatively minor. I did feel a little tearful after the surgery about the bruising and swelling on my face, and thinking, what have I done? But I was well looked after, and it was all well worth it. I had absolutely no regrets about my procedures and indeed would do it all over again. It took around two to three weeks for me to start seeing the results of the surgery, and I was already very happy. Some of the things I didn’t like about my face were caused by volume loss, and I was even more impressed with the outcome when I had the fillers to complete my new look. I felt so good about myself for the first time in years. When I look at before and after photos, I can’t believe what a difference there is in my appearance. When I had my procedure done, it was just after lockdown, and there were people that hadn’t seen me for a long time. I found that I would have to say hello to them as they didn’t recognise me at first. And when they realised it was me, they were totally gobsmacked at how well I was looking, but couldn’t actually put their finger on what was different. My partner was absolutely delighted at the results and how it made me feel. Friends and family that did not know that I had any procedure done were marvelling at how much younger I seemed to be. And when people ask my age, they have to ask again as they think they’ve misheard me. Scars are minimal and well hidden, and I feel absolutely fine about them. They don’t bother me in the slightest. Absolutely no regrets. I would do it all again in a heartbeat. I would tell anyone thinking of having the procedures to go for it, as it has been a life-changer for me. I felt totally taken care of from start to finish during my treatment, and the aftercare was first class. I cannot recommend McKeown Medical enough. You’re welcome with open arms and are made to feel that you matter to everyone, from reception staff all the way through to the specialists. And especially Darren McKeown, who knows exactly what to do to give you a totally natural look. So much so that there was only one person that actually guessed I may have had cosmetic surgery.

Post-surgery, she felt incredible, seeing daily improvements in her appearance. Now, she no longer hides behind makeup or avoids the camera; she embraces life with a radiant, youthful glow!

Transcript

Hi, I’m Lorna. I’m 56 years old, and I had a facelift with eyelid surgery, and I later came back for full face resurfacing. Where do I start? I felt my face looked a lot older than it should, and my lower face was starting to go south. My upper eyelids were heavy, and I looked tired all the time. I’d also spent 11 years in the States, and my face showed signs of sun exposure with wrinkles and that crepey-looking skin. My appearance was affecting certain aspects of my life, to be honest. Always wore my hair down as I felt more comfortable sort of hiding under it. And I’d try to disguise the way my face looked by applying makeup, but that only helped to a certain degree. In the last few years, I’ve hated having my pictures taken, even avoiding family photographs. In general, how I looked left me feeling down at times. I wanted to feel more confident in myself. I didn’t want to allow my life to be impacted by what I was seeing in the mirror. To be honest, I can’t remember worrying too much before any of my procedures. I guess the odd concern about what the end result would be like. Will it improve my appearance? Will it be painful? What about the recovery period? Probably the normal thoughts that most people have. But I knew I had to go through that to get to where I wanted to be. I was a little bit apprehensive. I’ve never had these types of procedures before, and I had very little knowledge of what to expect. I remember thinking, would it be made welcome? Would I be able to explain how I felt? Would the foot feeling awkward? Would everything be really clinical and formal? My fears disappeared as soon as I set foot in the clinic. Everyone was so warm, so welcoming. Dr Russell and Dr Darren were so professional and friendly. They listened to my concerns, and I had a few. I had to talk through everything, including any anxieties I had. I knew that all surgical procedures came with risks. But these were thoroughly explained by Dr Russell and Dr Darren. Everything was explained at a level that I could understand, and although everyone worries about what could go wrong, I felt that the team would do everything they could to reduce the chances of that happening. Both procedures went really well. With the facelift, I had the obvious swelling, bruising, surgical incisions. With the laser skin resurfacing, I was given local anaesthetic jabs to numb the whole area. The jabs were actually less stingy than I thought they would be. My face bled slightly, the skin oozed for the first 24 hours. But this had been explained to me prior to it, so I knew what to expect. I didn’t experience any pain during the procedure. I felt surprisingly good, to be honest. Prior to the procedures, I wasn’t really sure how I’d feel. I had in the back of my mind that it was bound to be uncomfortable. Any discomfort I did feel afterwards was minimal and disappeared by taking Paracetamol. I know it might sound sad or perhaps maybe a bit weird, but after both procedures, I woke up each morning looking forward to seeing the ongoing changes and improvements. It was amazing how one day can make a big difference in terms of changes. I didn’t feel down after any procedures. Probably felt a bit impatient more than anything else. As things take time to heal so the end results aren’t immediately visible. I’ve got no regrets at all about getting any of it done. Any of the procedures. I just wish I’d done it sooner. With the facelift and the upper eyelid surgery, I first noticed the difference in a couple of days. There’s obviously a degree of swelling and bruising for a couple of weeks, but I could still see the improvement. With the laser skin resurfacing, the skin was quite raw. It oozed. It was swollen in the days following the procedure. Therefore, it took me about a week to start seeing the results, and each day after, I could see further improvements. I feel great about my appearance now. I’m more confident. I’m not trying to disguise how I look by overdoing my makeup or avoiding certain situations. I’m absolutely delighted with the results. And I feel so grateful that I had such amazing doctors undertaking the procedures and looking after me. The differences are night and day. There are some photographs taken in the last few years where I look at myself and think how awful I look, to be honest. In some pictures, I look more like my parents’ age. Some even my grandparents’ age. Now, when I look in the mirror, I see on the outside how I genuinely feel on the inside.Numerous people have commented on my appearance since the procedures. I hadn’t told many people I was having it done. I’ve heard people say I looked refreshed, glowing, and healthy. Some have said it’s taken years off my age, which is really kind. My friends and family have always been supportive and would tell me I didn’t need anything done. But I wasn’t doing it for anyone else. I was doing it for me. Since I’ve had the procedures, they think I look great, and they see a big difference in my appearance and my confidence. I rarely think about my scars now. I’m not conscious that I have any. I know they’re there if I look for them, but it does not bother me at all. No regrets whatsoever. I just wish I’d met Dr Russell and Dr Darren sooner and had the procedures earlier. I would say get Dr Russell and Dr Darren to undertake their treatments as their consummate professionals and the best in their fields. Speak to your consultant first and don’t be afraid to ask questions. Look at before and after pictures, read the reviews, and people’s experiences. Make sure you take time to recover and heal, but ultimately, do it for yourself. I felt really well taken care of after my procedures. I had frequent post-op appointments, some daily, some weekly, and some monthly. I had 24/7 telephone contact should I require indirect access to the clinical team, should I need to ask anything. The whole team were always encouraging and reassuring and were there if I needed anything, no matter what it was. Whether it was just a small question or just some reassurance. My experience was second to none. From the moment I walked into that clinic, from my first visit, I felt relaxed in the serene atmosphere. I was looked after by the most professional and friendly guys and girls, who never failed to put me at ease with their smiling faces and willingness to help. Dr Russell and Dr Darren, I have no words to convey how grateful I am to them and how they’ve made me feel. With their knowledge, skill, professionalism, and kindness, backed by an incredible team alongside them. It has changed my life, and I cannot thank them enough.

Deep plane face & neck lift

From £25,750

How it Works

Step 1: Initial Consultation

Your consultation is thorough and unhurried. We assess facial structure, skin elasticity, and neck anatomy, and discuss your goals honestly. Clear guidance is essential.

Step 2: Surgical Planning

Your Deep Plane Face and Neck Lift is carefully planned. The degree of lift and tissue repositioning is tailored precisely to your needs.

Step 3: Pre-Op Preparation

Performed under local anaesthetic with deep sedation, the Deep Plane Face and Neck Lift allows controlled release of retaining ligaments and precise repositioning of deeper tissues. Excess skin is conservatively redraped without tension.

Step 4: Surgery

Deep plane face and neck lift surgery is performed under local anaesthetic with IV sedation. This means you sleep comfortably throughout your procedure, carefully monitored by a consultant anaesthetist, whilst your surgeon delicately performs your procedure. A typical surgery takes 6-8 hours of meticulous work, and you will be the only patient operated on that day.

Step 5: Recovery & Follow-Up

Most patients allow approximately two weeks away from work. Swelling and bruising peak early and gradually settle. We review you the following day, at one week, two weeks, and again at two and six months to ensure healing progresses as expected. We will see you for as many additional appointments if you need them during your recovery.

Recovery is typically more comfortable than anticipated.

Before, During, and After Deep Plane Face & Neck Lift

This patient is just a week out from her facelift surgery so she is still bruised, but you can already see the improvement.

Before, During, and After Deep Plane Face & Neck Lift

What To Expect Before Deep Plane Face & Neck Lift

Before surgery, you will attend a detailed consultation to assess facial structure, skin quality, and neck anatomy. We discuss your goals openly and provide clear guidance on what can realistically be achieved. We will write to you following your consultation with a detailed description of your consultation and treatment recommendations. You will receive thorough pre-operative instructions to ensure you are fully prepared.

What To Expect During Deep Plane Face & Neck Lift

Your Deep Plane Face and Neck Lift is performed in our HIS-registered clinic under local anaesthetic with deep sedation, ensuring you remain comfortable and relaxed throughout.

Consultant Plastic Surgeon Mr Russell Bramhall begins by placing carefully concealed incisions around the ears and within the hairline so they heal discreetly. The procedure then progresses beneath the SMAS layer, where the retaining ligaments of the face are released. This allows the deeper tissues to be repositioned properly, rather than tightened.

Where required, the neck is refined to improve contour and definition. The skin is then conservatively redraped without tension, and any excess is removed with precision.

The emphasis throughout is on repositioning, not pulling, restoring structure in a way that looks natural and proportionate.

What To Expect After Deep Plane Face & Neck Lift

Swelling and bruising are expected in the first two weeks and gradually settle. Most patients allow around two weeks away from work. We review you the day after surgery and regularly thereafter to ensure healing progresses smoothly. Final refinements continue to develop over several months as tissues settle into their new position.

Common Questions

What are the benefits?

Although the procedure involves lifting some of the skin from the cheek, a facelift procedure usually has only a minor impact on the cheek itself. Likewise, deep lines and wrinkles around the mouth area are rarely improved by this procedure in the long-term. This is because the changes that occur in this region are predominantly related to volume loss, so they are best treated using restorative techniques, such as injectable fillers, which are often combined with a surgical facelift. The main impact of the surgical procedure is to reduce excess tissue from the face, which is most notably seen on the lower face, jawline, jowls, and neck.

Which anaesthetic is used?

A Deep Plane Face and Neck Lift is performed under local anaesthetic with deep sedation which means you sleep comfortably throughout, monitored closely by a consultant anaesthetistBecause you’ve not had a full general anaesthetic, it means you recover much quicker with fewer side effects from drugs. Most patients feel well enough to go home just an hour or two after their surgery ends.

What's the downtime?

Most people will require a couple of weeks off work following the procedure, to allow the immediate signs of surgery to settle. We do have some patients who choose to return to work after a week or so, but we prefer you to take two weeks off if you can. We do not routinely use drains or bandages following surgery, thus enhancing patient comfort.

When will I see results?

We ask you to come in the day after your procedure for your first check-up. Following this, we would normally see you back in the clinic for a routine follow-up at one week, two weeks, six weeks, three months, and six months, plus, of course, as many other times as you require until we are all satisfied that you have fully healed and achieved the outcome we were anticipating. The scars from a Deep Plane Face and Neck Lift can take a year or more to fully settle and for any numbness to recover; however, we will keep you under our care until you are fully satisfied with the outcome.

The scars from a Deep Plane Face and Neck Lift can take a year or more to fully settle and for any numbness to recover; however, we will keep you under our care until you are fully satisfied with the outcome.

Will my face look tight or overdone after surgery?

No. A properly performed Deep Plane Face and Neck Lift should never make you look tight or pulled. Because the technique repositions deeper facial tissues rather than tightening the skin, it avoids the tension that creates an artificial appearance. The aim is structural restoration, not distortion. Most patients are told they look refreshed or well rested, rather than surgically altered.

Is a deep plane facelift suitable for both men and women?

Yes. A Deep Plane Face and Neck Lift is suitable for both men and women experiencing lower facial ageing. While surgical technique is adapted to preserve masculine or feminine facial characteristics, the underlying principle remains the same: restoring structural support and definition to the jawline and neck without compromising identity.

How soon can I exercise after a deep plane face and neck lift?

Light walking is encouraged in the first two weeks to support circulation. However, more strenuous exercise, including gym workouts, running, or heavy lifting, should usually wait four to six weeks.

Allowing adequate time before resuming intense activity helps minimise swelling and protects the quality of your final result.

Does a deep plane face and neck lift improve the neck as well as the face?

Yes. The Deep Plane Face and Neck Lift is specifically designed to treat both the face and the neck. By accessing deeper neck structures where required, we can improve laxity, contour, and definition in a comprehensive way. This ensures the jawline and neck appear balanced, rather than treated in isolation.

Why is surgeon experience important for deep plane facelift surgery?

The deep plane technique is anatomically complex and technically demanding. Releasing retaining ligaments safely and repositioning tissues precisely requires advanced training and a thorough understanding of facial anatomy. Experience directly influences not only safety, but also the subtlety and longevity of the result. In facial surgery, precision matters.
