Why the Right Treatment Varies So Much From Person to Person and What’s the Best Way to Address It

Weight loss can be life-changing. Whether you’ve achieved it through strict dieting, bariatric surgery or medications such as Mounjaro® or Wegovy®, reaching your target weight is a huge achievement. Unfortunately, many people discover that losing the weight doesn’t always leave them with the body they expected. Instead, they are left with loose skin, crepey skin or areas that simply don’t tighten in the way they had hoped. One of the biggest misconceptions is that there is a single treatment for loose skin that will fix all of your problems, which isn’t quite how it works. The best treatment option depends on what has actually changed, the severity of the changes and what your priorities are for improving.
Patients often use the phrase “loose skin” to describe several completely different problems.

Sometimes it’s a combination of more than one of these. These all require different treatments. One of the most important parts of our consultation is identifying which of these changes is affecting you and what your priories are for improving.
The earliest changes following weight loss are often surprisingly subtle. Rather than large amounts of hanging skin, many patients simply notice that their skin feels less firm than it used to. It no longer “snaps back” in the same way and may appear slightly loose on areas such as the upper arms, abdomen or around the knees. At this stage, the skin has lost some of its elasticity, but there is usually very little excess skin present.
For these early changes, Sofwave™ is often an excellent place to start. Sofwave™ uses advanced ultrasound technology to deliver precisely focused energy into the layer of the skin responsible for producing collagen. This controlled heating stimulates the body’s natural healing response, encouraging the production of new collagen and elastin without damaging the surface of the skin.
The treatment itself takes less than an hour, requires little to no downtime and allows you to return to normal activities immediately afterwards. Because Sofwave™ works by stimulating your own collagen production, the improvement develops gradually. Most patients notice their skin becoming progressively firmer over the following three to six months as the newly formed collagen matures and contracts.
Sofwave™ is particularly effective for patients with mild skin laxity, where the skin has started to lose its firmness but remains in relatively good condition. As the changes become more advanced, however, collagen stimulation with ultrasound alone may not be enough to achieve the level of improvement patients are hoping for.
As weight loss becomes more significant, many patients develop a different problem altogether. Rather than simply loose skin, the skin itself becomes thinner, weaker and develops a crepey, tissue paper-like appearance. This is especially common following rapid weight loss, including in patients taking GLP-1 medications such as Mounjaro® or Wegovy®, where the loss of underlying fat can leave the skin looking fragile and aged.
This is where Sculptra® often comes into its own. Unlike traditional dermal fillers, Sculptra® does not work by adding volume. Instead, it stimulates your body’s own collagen production from within the skin itself. Over the following weeks and months, the skin gradually becomes thicker, stronger and more resilient as new collagen is laid down. Rather than simply tightening the skin, Sculptra® improves the quality of the skin itself, making it feel healthier and appear smoother and less crepey.
Because collagen production takes time, the results develop gradually, with improvement continuing for several months following treatment. A course of treatments is usually recommended to achieve the best outcome, with the effects continuing to evolve long after the final session.
In our experience, patients whose main concern is crepey skin often achieve a more noticeable improvement with Sculptra® than with Sofwave™ alone. That’s because the primary problem is no longer simply skin laxity—it’s the quality of the skin itself. For many patients, the best results are achieved by combining Sculptra® with Sofwave™, allowing us to improve both the quality of the skin and its firmness. Patients with more significant skin laxity, however, may require a more powerful skin tightening procedure such as QuantumRF®, which we’ll discuss in the next section.
As skin laxity becomes more advanced, there comes a point where non-invasive treatments such as Sofwave™ are simply no longer capable of producing enough tightening. The skin has stretched beyond its ability to respond to surface-based collagen stimulation alone, yet there may still not be enough excess skin to justify surgery. This is often the “middle ground” where patients want a more noticeable improvement but are keen to avoid the scars and recovery associated with a surgical body lift.
For these patients, QuantumRF® can be an excellent option. Unlike Sofwave™, which delivers ultrasound energy from outside the body, QuantumRF® works beneath the skin. Through a series of tiny puncture sites—typically only a few millimetres in size—a fine treatment probe is passed underneath the skin to deliver precisely controlled radiofrequency energy directly into the tissues where tightening is required. Because the energy is delivered internally rather than through the surface of the skin, much higher temperatures can be achieved safely, resulting in significantly greater collagen contraction and skin tightening.
The treatment is performed under local anaesthetic, and because the access points are so small, there are no long surgical scars. Instead, patients are left with tiny puncture marks that usually heal extremely well and become barely noticeable over time. For many patients, this offers an attractive balance between the modest improvement achieved with non-invasive treatments and the more dramatic, but more invasive, results of surgery.
One of the advantages of QuantumRF® is that the tightening effect continues to improve long after the procedure itself. While some tissue contraction occurs immediately, the body continues to produce new collagen and remodel the treated tissues over the following three to six months. During this time, patients typically notice progressive improvement in skin firmness as the collagen matures and contracts.
Although QuantumRF® can achieve substantially greater skin tightening than Sofwave™, it is important to understand that it still cannot remove more severe excess skin. Once the skin has stretched beyond a certain point, the only way to eliminate it completely is through surgery. However, for patients with moderate skin laxity, QuantumRF® often fills an important gap, providing a meaningful improvement without the scars and recovery associated with procedures such as an arm lift or tummy tuck.
For many patients, loose skin isn’t the only issue. Even after significant weight loss, small pockets of stubborn fat often remain beneath the skin, particularly on areas such as the upper arms, abdomen, flanks, thighs and around the knees. This creates a unique challenge, because treating only one part of the problem rarely produces the best result.
If the skin is tightened without addressing the remaining fat, the area may still appear bulky or poorly contoured. Equally, if the fat is removed without tightening the skin, the reduction in volume can actually make loose skin appear even more noticeable. This is why successful body contouring is about much more than simply removing fat – it requires a balanced approach that addresses both the fat and the skin together.
This is the principle behind Quantum™ Body Contouring. Rather than relying on a single technology, the procedure combines three complementary treatments, each designed to address a different aspect of body contouring.
The first stage uses Quantum™ Ultrasonic Liposuction (UAL). Ultrasound energy gently breaks down and emulsifies fat before it is removed, making fat extraction more efficient while preserving the surrounding tissues. This allows for smoother contouring and can be particularly helpful in areas where the fat is more fibrous or difficult to remove using traditional liposuction techniques.
Once the fat has been softened, Power-Assisted Liposuction (PAL) is used to precisely remove it. PAL uses a rapidly vibrating cannula that allows the surgeon to sculpt the body with greater control and accuracy than conventional liposuction alone. This helps create smoother, more natural body contours while minimising trauma to the surrounding tissues.
The final stage is QuantumRF® Skin Tightening. After the unwanted fat has been removed, radiofrequency energy is delivered beneath the skin to stimulate immediate collagen contraction and long-term collagen remodelling. This encourages the skin to tighten around its new contour, helping to reduce the risk of residual skin laxity and producing a firmer, more refined result over the following three to six months.
By combining all three technologies into a single procedure, Quantum™ Body Contouring allows us to address the two main consequences of weight loss—stubborn fat and loose skin—at the same time. For many patients, this provides a level of contour improvement that would not be possible by treating either problem in isolation.
It is important to recognise, however, that even Quantum™ Body Contouring has its limits. If there is a very large amount of excess skin, no skin-tightening technology can make that skin disappear completely. In those situations, surgery to remove the excess skin may still provide the best outcome, which we’ll discuss in the final stage of the treatment ladder.
There comes a point where no amount of collagen stimulation or skin tightening can replace what has been lost. Following major weight loss, the skin can become so stretched that it simply has too much surface area to shrink back, regardless of the technology used. At this stage, the issue is no longer one of skin quality or elasticity—there is simply too much skin.
This is one of the most important conversations we have with patients. While modern skin tightening technologies have transformed what we can achieve without surgery, sometimes there is just so much excess skin that the only option is surgical excision. The less invasive technologies can improve the quality of the skin, encourage it to tighten and contract, and in many cases delay or even avoid the need for surgery. However, when large folds of skin are present, no non-surgical or minimally invasive treatment can make that skin disappear.
For patients with significant skin excess, body contouring surgery remains the most effective treatment. The exact procedure depends on the areas affected. An arm lift (brachioplasty) removes loose skin from the upper arms, creating a slimmer, firmer contour. A tummy tuck (abdominoplasty) removes excess abdominal skin and, where appropriate, repairs separated abdominal muscles to restore a flatter, stronger abdomen. For patients with loose skin affecting the upper legs, a thigh lift removes excess skin and reshapes the thighs to improve both appearance and comfort.
Although surgery involves larger scars and a longer recovery than non-surgical treatments, it also produces the most dramatic improvement for patients with advanced skin laxity. The scars are carefully planned and positioned to be as discreet as possible, and for many patients they are a worthwhile trade-off for the improvement in body shape, comfort and confidence that can be achieved.
One of the advantages of offering the full spectrum of treatments is that we are not committed to recommending surgery simply because we have surgeons who do the procedures — or recommending non-surgical treatments simply because we own the technology. Instead, we assess the severity of your skin changes and recommend the treatment that we genuinely believe is most likely to give you the result you are hoping for. For some patients that will be Sofwave™ or Sculptra®. For others it will be QuantumRF® or Quantum™ Body Contouring. And for those with significant excess skin, surgery may simply be the most honest and effective solution.
The key is choosing the treatment that matches the problem. That’s why the most important part of your journey isn’t selecting a technology—it’s receiving an expert assessment that identifies exactly what is causing your concerns and the treatment most likely to achieve your goals.
Over the past few years we’ve seen a significant increase in patients seeking treatment for loose skin following weight loss with GLP-1 medications such as Wegovy® and Mounjaro®. These medications have transformed the treatment of obesity, helping many people achieve weight loss that would previously have been extremely difficult. For many patients, the improvement in their health, confidence and quality of life has been life-changing.
It’s important to understand, however, that GLP-1 medications do not damage the skin. The skin changes people experience are primarily a consequence of losing weight, particularly when that weight loss is significant or occurs over a relatively short period of time. In many ways, these are the same changes we have always seen following successful weight loss through diet, exercise or bariatric surgery. The difference is simply that many more people are now achieving substantial weight loss than ever before.
Our skin is remarkably elastic, but it is not infinitely elastic. Over time, being overweight stretches the skin to accommodate increased body volume. When weight is lost, the underlying fat disappears much more quickly than the skin can remodel itself. Collagen and elastin—the proteins responsible for the skin’s strength, thickness and elasticity—take months to regenerate, and in some cases they are simply unable to recover sufficiently to match the body’s new shape.
Another important factor is the loss of structural support beneath the skin. Fat doesn’t just increase body size; it also provides support to the overlying skin. As that fat disappears, particularly following substantial weight loss, the skin can appear thinner, less supported and more prone to wrinkling or sagging. This is why some patients notice crepey skin, loose upper arms or abdominal skin that seems to have “deflated” after losing weight.
The likelihood of developing loose skin varies enormously from person to person. Age plays an important role, as younger skin generally has a greater capacity to recover than older skin. Genetics also influence how elastic your skin is, while factors such as smoking, long-term sun exposure and the total amount of weight lost can all affect how well the skin retracts. In general, the greater and more rapid the weight loss, the more likely it is that skin changes will become noticeable—but there are many exceptions, and some patients experience very little loose skin even after losing a substantial amount of weight.
The good news is that loose skin after weight loss is no longer a problem with only one solution. Today’s treatments range from collagen stimulation and non-invasive skin tightening through to minimally invasive procedures and surgery. The key is identifying which type of skin change you have, because the best treatment isn’t determined by how you lost the weight—it’s determined by what has happened to your skin.
One of the biggest mistakes we see is patients trying to choose a treatment before they’ve understood the problem they’re trying to solve.
Many people begin by searching online for terms such as “best skin tightening treatment” or “how to get rid of loose skin after weight loss.” They read about a particular technology, see impressive before and after photographs, and assume that treatment must be the right answer for them.
The reality is that loose skin isn’t a diagnosis—it’s a symptom. Two people may describe exactly the same concern, yet require completely different treatments. One patient may have thinning, collagen-depleted skin that would respond best to collagen stimulation. Another may have moderate skin laxity requiring a minimally invasive skin tightening procedure. A third patient may have residual fat beneath the skin, making body contouring the best option. And for someone with significant excess skin following major weight loss, surgery may simply be the most effective and predictable solution.

The Quantum Lift!
We introduced this new technique only a few weeks ago and, well, it’s going even better than expected. This is one of our patients before and immediately after. I always warn my patients that I cannot give them a surgical facelift result with a procedure that doesn’t involve cutting but honestly, in this case, I think we got pretty close to a facelift result and closer than I was expecting.
First we remove some fat from under the chin and jawline before the quantum goes in to tighten the skin. I have included a picture of the neck after the we had removed fat from both sides, but only applied the quantum to her right side. You can see how much difference there is when I try to pinch the skin. In the non-treated skin I skin still pinch an inch (or two) but on the treated side there is very little to pinch, because the tissues have shrunk down so much.
The patient tolerated the procedure very well under simple local anaesthetic without sedation and we had a nice chat throughout. Afterwards she had a cup of tea and then went home with her husband half an hour later.
I have been very excited about The Quantum Lift for a while, but I’m only getting more excited the more I do. I really believe this is a massive step towards an effective way to rejuvenate the lower face and neck whilst avoiding traditional facelift scars.
Everyone knows we do lots of sophisticated facial work in the clinic. But did you know we also now do a lot of breast work too?
This is one of our patients who wanted breast augmentation. She specifically wanted more upper pole fullness so Kavita selected a 325cc high profile implant from Mentor, but placed them under the muscle to give a more natural take off. Mentor are considered the gold standard for breast implant quality and we only use their implants in the clinic.
The patient is just a few weeks post-op, so there is still a little bit of settling to go but she is already delighted with the shape and size. The key to breast surgery is listening to the patients concerns and then tailoring the procedure to get them the result they want - which is something the very fabulous Miss Kavita excels at.

So here she is! The one we’ve all been waiting for 😍 This is Lorraine 5 days after her Quantum lift. She’s still at home recovering because she had her body done too (more on that soon), so she’s not been back in the clinic for proper after photos yet. But she’s already delighted and sending the dreamteam these selfies from home. She’s obviously still very early and a bit swollen, but I think we can all see where this is heading already! This patient is 62 and came to us feeling self-conscious about the heaviness and puffiness of her upper eyelids. She`d found herself regularly raising her eyebrows just to lift the skin away from her eyes and had noticed a shadow appearing when she looked upwards and outwards, which was beginning to affect her peripheral vision.
Heavy upper eyelids can really knock people`s confidence, especially in social situations and in photos, and yet it`s one of the most straightforward areas to address.
The procedure itself takes around 45–60 minutes and is carried out while patients are awake under local anaesthetic. The scars sit within the natural crease of the eyelid, making them almost impossible to spot once healed
This is just 2.5 months post-surgery, and her scars are already settling beautifully. They`ll continue to fade over the next 12 months.

The deep plane face and neck lift is one of the most technically challenging procedures in plastic surgery. It takes 6-8 hours of painstaking dissection to carefully release the retaining ligaments of the face so the face can be properly lifted and repositioned, rather than simply pulled tighter around the edges.
Few surgeons have the technical skill to pull this off - and we are very proud of @bramhallplasticsurgery for being one of them. It’s a challenging procedure to get right, but when the results are this good, it’s definitely worth it!

Varicose veins are something we see a lot of in the clinic, and they can have a real impact beyond how they simply look - aching, heaviness and discomfort are common reasons for patients deciding to come and get them fixed. Now that summer is approaching, this our peak time of year for patients coming to look for a fix.
This patient was treated by the wonderful Dr Alex Vesey using a combination of Radiofrequency Ablation (RFA) and microphlebectomy. RFA works by delivering targeted heat energy to close off the affected vein from the inside, while microphlebectomy allows the smaller surface veins to be removed through tiny incisions. Together, they address both the underlying cause and the visible result. Both procedures are done under local anaesthetic, which means you can get up and walk out immediately after surgery.
