Wrinkle-relaxing injections remain one of the most effective ways to soften signs of ageing – particularly in the upper face. Wrinkle-relaxing injections have transformed the way we treat facial ageing – particularly in the upper face. By carefully placing small amounts of botulinum toxin into specific muscles, we can soften the appearance of lines and restore a more relaxed expression.
Relfydess, our newest generation of anti-wrinkle treatment, is derived from a different strain of the botulinum bacteria and early clinical trial data suggests it offers longer-lasting results than traditional products. In clinical trials, around 75% of patients were still seeing visible improvement at six months. As one of the first clinics in the UK to work with the new product, including carrying out clinical trials, we decided to incorporate it into our practice early particularly as an option for patients who have had poor results with previous toxins.
The only negative effect we have seen from Relfydess so far is that it seems to be more painful on injection. The needle calibre is the same, so the initial pin prick feels the same – but there is a sting as the drug is injected under the skin. If the longevity is as good as indicated by the trial data then this might be a worthwhile price for the enhanced duration of effect.

Treatment of mild frown lines.
The frown lines are one of the most commonly treated areas using wrinkle relaxing injections.
The muscles that we use to frown also depress the brow. By relaxing them and preventing that effect, we can simultaneously lift the brow, making the eyes look more open and awake
Treatment of forehead lines.
The forehead is another area that is commonly treated with wrinkle relaxing injections.
The forehead muscle – the frontalis – is the only muscle we have for lifting the brow. If we relax it completely, it will cause the brow to drop. It is therefor important to only treat a section of the forehead muscle with anti-wrinkle injections, leaving enough muscle movement to support the brow. Achieving this is a delicate balance and is dependant on the skill and experience of your doctor.
In some cases, where there is significant excess skin and heavy, hooded upper eyelids, it is best not to perform anti-wrinkle treatments on the forehead at all.
Treatment of crow's feet using a combination of wrinkle relaxing injections and cheek filler
Crow’s feet can be effectively treated with a wrinkle relaxing treatment.
This works especially well for women in their 30s and 40s. When we get into our 50s, however, the muscles around the eyes become very hyper-expressive compounded by loss of volume from and descent of the cheek.
Our preferred way of addressing this issue is to combine wrinkle relaxing injections with some filler in the cheek. This not only improves the reduction of crows feet, but can also help to increase the time between repeat treatments by using a combination approach.

The depressor anguli ori (or DAO for short) is a muscle that goes from the corner of the mouth down towards the jaw.
Talking, eating and certain facial expressions cause it to contract, which pulls the corners of the mouth downward. Relaxing it with anti-wrinkle injections has the opposite effect — it elevates the corner of the mouth. This can be quite a difficult effect to achieve and is best suited to younger patients with fine lines.
Lip fillers and fillers for lipstick lines are another way to focus on the mouth area.
The superficial muscle of the neck, the platysma, usually contracts to pull the neck and lower face downwards. Treating the neck with wrinkle relaxing injections can help to relax these thick bands, as well as relieve the negative pull on the lower face and neck.
This is not an anti-wrinkle treatment that is suitable for everyone. It is best suited to younger patients who have a thin neck.
Other options are CoolSculpting and a non-surgical facelift.
With Relfydess, many patients continue to see visible softening of lines for up to six months – significantly longer than traditional wrinkle-relaxing treatments. While muscle movement may gradually return before then, there is often a delay before lines begin to reappear. It’s typically at that stage that a top-up treatment is advised.
Most patients find they only need to have treatment twice a year to maintain their results, making Reflydess a more convenient and longer-lasting option in the long term.
We often combine Relfydess with other non-surgical treatments – such as hyaluronic acid dermal fillers or targeted laser procedures – to achieve the most natural and balanced results. In many cases, the best outcomes come from a thoughtful combination approach, rather than relying on a single treatment alone.
Treatment with Relfydess is generally very well tolerated. Injections into the upper face are almost pain-free, thanks to the use of ultra-fine needles and skilled technique. Bruising is uncommon, and most patients are able to return to normal activities immediately after treatment.
The most frequently reported side effect is a mild headache, which may affect up to 1 in 10 patients. In most cases, this resolves within a day or two, though occasionally it can persist for longer. These headaches are typically mild and do not usually require any medication.
As with all botulinum toxin treatments, there is a small risk of the product affecting nearby muscles. In the upper face, this can occasionally result in temporary drooping of the eyelid or eyebrow, though such effects are uncommon and usually resolve within a few weeks. When used in the lower face or neck, there is a small risk of temporary weakness in muscles that control the mouth or speech – though these effects are rare and also temporary.
