For many women, periods are more than just a monthly inconvenience. They can be genuinely debilitating. Heavy bleeding, painful cramps, bloating, fatigue, and disruption to daily life are common experiences, yet often dismissed or downplayed. One of the most frequent treatment options offered is the contraceptive pill. And while this may help some, it’s certainly not the only – or always the most appropriate – solution.

Common causes of heavy or painful periods

Not all period pain or heavy bleeding is ‘normal’. In fact, these symptoms can be signs of underlying conditions that require proper investigation. Some of the most common causes include:

  • Hormonal imbalances, such as those related to oestrogen and progesterone
  • Uterine fibroids – benign growths in the uterus that can increase bleeding and discomfort
  • Adenomyosis – a condition where the inner lining of the uterus breaks through into the muscle wall, causing painful, heavy periods
  • Endometriosis – where tissue similar to the womb lining grows outside the uterus
  • Polyps or other structural abnormalities within the uterus

Holistic treatment and lifestyle changes

There are ways to support your menstrual health through lifestyle, and while they may not be a cure, they can be a helpful part of a wider management plan.

  • Diet: Anti-inflammatory foods, maintaining stable blood sugar, and avoiding excessive caffeine or alcohol may ease symptoms.
  • Exercise: Regular movement can reduce bloating and cramps, and improve overall mood and energy.
  • Stress management: Chronic stress can worsen hormonal imbalances, so stress reduction techniques like yoga, meditation or counselling can play a role.
  • Iron supplements: Important for those experiencing heavy bleeding, to prevent anaemia and fatigue.
  • Pain relief: Over-the-counter NSAIDs such as ibuprofen can be helpful for managing cramps.

These methods can offer short-term relief, but if you’re continuing to suffer month after month, further treatment options should be explored.

Hormone-based treatment options

Beyond the pill, there are several other hormone-based treatments available:

  • Hormonal IUDs (e.g., Mirena®): These release progesterone locally in the uterus, often reducing or stopping periods altogether. They’re effective but may not be suitable for everyone.
  • Progesterone-only pills or injections.
  • GnRH analogues, used more short-term for conditions like endometriosis.

Pros:
Often effective in reducing bleeding and pain, non-surgical

Cons:
Potential side effects (mood changes, bloating, breakthrough bleeding), and not ideal for those seeking hormone-free options or those who have had poor experiences with hormones in the past.

Some patients are simply looking for non-hormonal treatments. Others may have contraindications to hormonal therapies. For these individuals, exploring alternatives becomes a priority.

Surgical options: When is it time to consider more?

If you’ve tried medication and lifestyle changes but are still experiencing significant symptoms, surgical interventions may offer more lasting relief.

  • Endometrial ablation – destroys the uterine lining to reduce or stop bleeding, without removing the uterus.
  • Hysteroscopic removal of fibroids or polyps.
  • Hysterectomy – complete removal of the uterus, typically only considered when all other treatments have failed.

We’re proud to be the first clinic in Scotland to introduce Relfydess – a new generation of wrinkle-relaxing treatment.

We performed the first Relfydess treatment in Scotland for one of our patients: the technique is similar – but the science is a little new.

Relfydess is derived from a different strain of bacteria. In clinical trials, around 75% of patients were still seeing visible improvement at six months which is longer than we would normally expect, which is why I’ve been so excited by this.

I think this one is going to be especially interesting for patients who’ve been having treatment for a while but not getting the same longevity.
